Sports Clearance Packet

Sports Clearance Packet Students must complete the following steps in order to try-out or participate in any sport:    

1. Complete the online registration through our athletic website.  
2. Attach this packet Home Campus registration  
3. Parent and student MUST sign all documents requesting a signature.

• Sports Physical / Screening Form
